The DIAbetes MANagement and Treatment (DIAMANT) cohort is a population-based, dynamic, prospective real-world dataset built from electronic general practitioner (GP) information systems in the Netherlands, designed to generate timely, “real-life” insights on diabetes prevalence, treatment patterns, safety, effectiveness, and outcomes—while enabling comparison to matched controls without diabetes and linkage to other (national) datasets.

Research challenge

Diabetes prevalence continues to rise, while long-term safety and effectiveness of both newer and established glucose-lowering therapies remain insufficiently understood in routine practice—creating a need for scalable pharmaco-epidemiological research grounded in real-world care delivery. The challenge was to develop a cohort that could provide quick, up-to-date evidence on prevalence, treatment, safety, and effectiveness using routinely collected primary care data.

The cohort needed to reflect a near-complete, population-based perspective (to support epidemiological measures and follow-up) while acknowledging the limitations of routine clinical data (e.g., missingness, variation in recording practices, and data not collected primarily for research).
